Check Signature Form

The attached check signature form is to be used for client signatures that will appear on checks, such as paychecks and agency payments.  

To ensure a high-quality signature for check printing, provide two sample signatures with the following items:
  1. Sign the signature as you wish for it to appear on your checks
  2. Ensure your signature is within the boundaries of the appropriate boxes
  3. In the event two individuals must sign your checks, provide the two individual signatures within the same double signature box provided below·      
  4. Use a bold, black pen for your signature 
  5. Do not submit a rubber-stamped signature
