To report on employees enrolled in benefit plans, navigate to Benefits / Benefit Plans
The following columns are helpful in this report:
Benefit Type -- Medical, Dental, Vision, etc.
Benefit Plan: Name -- Specific Plan Name within each Benefit Type
Coverage Name -- Coverage Level (Employee Only, Employee + Spouse, Family, etc)
Deductions 1 Effective From -- Payroll Deduction Start Date
Deduction 1 Effective To -- Payroll Deduction End Date
*For plans with multiple deductions, insert columns for Deduction 2, 3, etc.
Coverage Effective From -- Insurance Start Date
Coverage Effective To -- Insurance End Date
Plan Amount EE 1 -- Employee Payroll Deduction Amount
Plan Amount ER 1 -- Employer Payroll Deduction Amount
*For plans with multiple deductions, insert columns for Plan Amount 2, 3, etc.
